An engine bay is the hardest area on a car to make look right, because everything in it is visible from several angles at once and none of the surfaces are simple. It is also structural in a way people forget. Shock towers, inner wings and the apron are the front of the car's structure, and on most vehicles they are carrying suspension loads directly.
So engine bay work is two jobs at once. The structural repair has to be right because the suspension is bolted to it, and the finish has to be right because you are going to be looking at it every time the hood is open.
Shock towers, which crack around the top mount and rot at the base where they meet the inner wing.
Inner wings and aprons, which trap everything the front wheels throw at them and rot from the back where nobody looks.

Engine bay smoothing is a popular request and it is worth being honest about what it does. Removing unused brackets and filling redundant holes looks excellent. Doing it over rot, or removing something structural because it looked untidy, is a problem you will find later.
So the structure gets repaired first and the tidying happens after, with anything load bearing left alone or replaced rather than removed.
Finishing in a bay means dressing welds on surfaces that curve in two directions with limited access. It is slow and it is why engine bays get skipped or buried in seam sealer on cheaper work.

Yes, and it is common on older vehicles. Towers crack around the top mount because that is where suspension load concentrates. The repair reinforces that area rather than just closing the crack, since closing it puts you back where you started.
Filling redundant holes and removing brackets that are no longer used, so the bay reads as clean surfaces. It looks excellent when the structure underneath is sound and it is a bad idea when it is being used to tidy over rot.
For most structural work, yes, or at least most of it. Access in a full bay is the limiting factor on doing the job properly, and we would rather say that than do a compromised repair.
